Insurance remittance
Some Insurance companies send payment for laboratory testing services directly to the patient. These payments need to be remitted to LSA Lab, LLC by choosing either option below.
You can endorse the check(s) to “LSA Lab, LLC” and send it in the mail along with the E.O.B. in the pre-addressed envelope you received
ORYou can deposit the check(s) into your personal account and then click the “Submit Payment Now” button below to send payment directly to LSA Lab, LLC. Email a copy of the E.O.B. to EOB@lsalv.com so we may match it up to your payment(s). Please make the subject line - “Your Name, EOB mm/dd/yy” where the date is the date you were tested. If you have multiple test dates list as follows, “John Smith, E.O.B. 8/22/21, 9/1/21, 9/13/21”. If family members are covered as well put them in the body of the email.
When making payment online put use the invoice number from the E.O.B. under Invoice Number. If you don’t have the invoice number put the check #.
